Terms & Conditions
Disappearink is my trading name. These terms and conditions outline the rules and regulations for the use of my products, services and website. Disappearink is located at: Jubilee House, Morley, Leeds, LS27 9JQ, UK.
Terms of Service
These terms and conditions of service explain how I provide my service to clients. Please ensure that you’ve read and agreed to them before booking in with me.
Online Consultation
Any information you give to me via your online consultation must be a true representation of your current situation. I use this information to provide you with the most tailored, effective, and safe treatment possible.
New Clients (PRICES WILL BE INCREASING BY 5% from 1ST APRIL 2025)
Once you have booked online, filled out your medical form and paid your £30 deposit you will receive an automated email from acuity scheduling "Appointment Scheduled". Once I have checked your medical form I then email you a confirmation email with further information prior to your appointment. The £30 deposit will be deducted off your first full session. This deposit is non-refundable if you miss your appointment, and if you want to re-book you will need to pay in full in advance for all following sessions. You can only reschedule 48-hours prior to your consultation, no less.
Active Clients (PRICES WILL BE INCREASING BY 5% from 1ST APRIL 2025)
At the time of booking an appointment I will take a deposit £30 deposit to secure future appointments. This will be deducted off your next session price. You can re-schedule your appointment a minimum of 24 hours in advance. If you cancel less than 12 hours prior to your appointment, or do not show up to an appointment you will need to pay £30 again to book your next appointment. (Obviously there can be emergencies, and I will take each circumstance in to consideration)
I will do all I can to make sure that your session takes place at the agreed time. If I need to cancel or postpone your session, I will try to contact you in advance using the contact details you have provided to me. I may message you on the day of your appointment to offer an earlier or later time, if, for example; someone cancels last minute. However please do not feel obliged to accept unless it works well for you.
Reminders
Twilio via Acuity is used to send client email and text reminders. If you do not wish to receive emails you can unsubscribe by clicking the link in the first email you receive. For text reminders standard text messaging rates apply. Clients can opt out of receiving text messages by replying "STOP". Appointment reminders will now be sent by email/text 48-hours prior to your appointment. This gives you plenty of time to re-schedule or cancel your appointment so that I can offer your space to someone else. Please do this via your email rather than messaging me directly. (If you struggle then yes, please don't hesitate to contact me and I'll be in touch as soon as I can)
Treatment Sessions
Please arrive on time for your session. If you arrive in advance, I do not have a waiting room so please wait until your session time. You can try to buzz in early but please note I may have a client in the room and may not be able to answer the buzzer. If you arrive for your session after the agreed time, I will make every effort to accommodate you, but at busier times I may only be able to treat you up until the end of your agreed session time.
I reserve the right to refuse treatment for whatever I consider to be reasonable grounds, without refunding your deposit or the most recent treatment plan payment. Reasonable grounds may include but are not limited to, a tan or fake tan, a medical reason that prevents treatment taking place, lack of personal hygiene, or a security concern.
Before you attend your session, I will let you know what you need to do to prepare with pre-treatment instructions. If you don’t follow the instructions, then I may need to cancel your session.
I do not allow children in my treatment room.
Courses of Treatment
The frequency of treatments depends on the size and location of the treatment area. Treatment frequency following your first session will be decided by myself. All treatments are paid either in full in advance, per session, or in monthly instalments by bank transfer for larger tattoos. A course of treatment is only refundable for medical reasons.
Payment
To book a patch test with Disappearink a £30 non-refundable deposit is required. I reserve the right to charge you £30 if you miss the session and for cancelling a session with less than 48 hours’ notice of the session start time. If you come to your appointment as planned, this £30 will be deducted off your first full session.
I do not provide credit as defined under the Consumer Credit Act 1974. Paying for treatment monthly is a ‘pay as you go’ arrangement, where you pay in regular monthly instalments for a course of treatment. This is only usually needed for larger treatments. Treatments and sessions are non-transferable.
Suitability for Treatment.
All treatments:
For all treatments and patch test sessions, the area to be treated needs to be TAN FREE. This includes real tan, fake tan, and sun beds. (Please note: if you have EVER used tanning injections you can not have laser treatments). If you arrive for your appointment with a tan I will not be able to treat you and you may still be charged for your session. My decision to commence or cease treatment for whatever reasons is final. This means that I may not be able to treat you if I decide you are currently unsuitable to receive treatment, or if the treatment has reached its natural conclusion.
I use a medical consultation form and an in-person consultation to check whether treatment is suitable for you before I begin. If you are not suitable for treatment, I’ll let you know the reason(s). If I cannot treat you due to a medical reason, I will refund if you have paid a patch test deposit. If I have to stop treatment midway through a course of treatment, for medical reasons only, then I will offer you a refund.
Hair removal treatments:
For hair removal patch tests the area to be treated must not be waxed, plucked, epilated or bleached for at least 1 month before the start of the session. Shaving is allowed however please leave a small area of hair with hair growth at least 2-3mm in length so I can assess your suitability for treatment. Following your hair removal patch test session you must be fully shaven in the treatment area for your first full hair removal session or I will not be able to treat you.
Consent to Treatment
For treatment to take place, you need to agree to the following terms. On your first visit to Disappearink we will ask you to agree to these terms again before we start your treatment, to ensure nothing has changed between now and then.
For treatment to take place, you need to agree to the following terms. On your first visit to Disappearink we will ask you to agree to these terms again before we start your treatment, to ensure nothing has changed between now and then.
- The information you have given is correct to the best of your knowledge, and you have not withheld any known medical state or condition.
- You will inform the Laser/IPL operator before each treatment if there has been any changes (for example medications taken).
- You understand that the results from this treatment vary considerably and a small percentage of people will not respond satisfactory to treatment.
- You understand multiple treatments are necessary to achieve satisfactory results.
- You understand there is no guarantee of permanent results and maintenance treatments may be necessary (not including laser tattoo removal).
- You understand that you must avoid sun exposure on the treated area for the duration of the treatment (and for up to 1 month afterwards) or use a high sun protection factor to avoid sun damage. You understand that tanned skin cannot be treated.
- You understand that there may be short-term side effects such as reddening, bruising, swelling, mild burning or blistering, hypo-pigmentation, (lightening of the skin) or hyper-pigmentation, (darkening of the skin), as well as rare side effects such as scarring and permanent discolouration.
- You understand that pigmentation areas caused by sun damage may initially turn darker. This will be followed by ‘micro-crusting’ of the lesion, after which it should flake away leaving an area without excess pigmentation.
- You understand that you must wear protective eye wear to prevent damage from the light. We will provide this for you to wear during treatment, unless otherwise stated.
- You agree that your Laser/IPL operator may need to increase the fluence (intensity / strength of treatment) in order to get the results you expect.
- You give your consent for Disappearink to use any photos you have sent to us electronically, or otherwise given to us, to be used to clinically evaluate your treatment area.
- You certify that you have read and understood all the information and have answered any questions we have asked you truthfully before agreeing to treatment and consent to the terms of this agreement.
